A controversial surgery is a surgery not endorsed by the overall medical community as safe or healthy. If a controversial surgery is performed, it is usually by an individual doctor of questionable background, sometimes without a physician's license, in an obscure location designed to be hidden from the mainstream world in case of failure.
Controversial procedures are not endorsed by any reputable medical association until they are thoroughly tested and emerge legitimate, in which case controversy may still occur, but the phrase controversial surgery no longer applies because the surgery has been brought into mainstream medicine.
