138th Street–Grand Concourse is a station on the IRT Jerome Avenue Line of the New York City Subway, located at the intsersection of East 138th Street and the Grand Concourse in the Bronx neighborhood of Mott Haven. It is served by the 4 train at all times except rush hours in the peak direction, and by the 5 train at all times except late nights. It is the southernmost Jerome Avenue Line station in the Bronx. The station, which has crossovers from one wall platform to the other, also has mosaics indicating "Uptown Trains" and "Downtown Trains" in the mezzanine.
The original name of the station was Mott Haven Avenue, as is indicated by the "MH" mosaic border.
